_____YES! I want to be a Tama-Do Certified Practitioner! Sign me up for the Level I Test, which will happen 2 days before the beginning of the Level II course
(arrive by 4pm June 15, test all day June 16 and 17).

_____Yes! I understand that to become a Certified Tama-Do Practitioner I must:

• □ Take all 10 Level I Courses (which includes the Level I Review and Tama-Do Ethics, which is NOT offered in the Level I Summer Intensive, but in the Spring prior to Level II.
• □ Fulfill the Tama-Do Practice Protocol which includes handing in 50 client sessions PRIOR to the Level I Review and Test
• □ Successfully pass the Level I Test
• □ Take Level II Summer Intensive only offered once a year in Europe.
• □ Sign the Tama-Do Practitioner Code of Ethics and Conduct as well as the Level II RSVP contract with the Academy and the Level II registration form.

_____Yes! I understand that to retain my Tama-Do Practitioner Certification Status, I must return once a year for a Tama-Do Master Class or Shamanic Journey.

_____NO THANK YOU. I do NOT want to become a Tama-Do Certified Practitioner. I am taking Level II for my own personal development. I understand that in order to be able to participate in the Level II training, I must sign the Tama-Do Code of Ethics and Conduct.

_____Yes! I understand and accept the following Tama-Do Academy Policies:

□ Tama-Do Academy reserves the right to accept or reject the qualifications of any student to participate in any Tama-Do class, workshop, etc. Tama-Do further reserves the right, at any time, to rescind the participation of any student in a class, workshop etc, with no refund, if the student is disruptive to the energy of the Teacher, the Teachings or the group.

□ All Tama-Do Academy Summer Trainings are PRO- ENERGY FIELD. This means that we are SMOKE FREE, PERFUME FREE, DRUG FREE and ALCOHOL FREE as it affects the energy field. Any student found smoking, using recreational drugs or disturbing the peace with drinking will be expelled immediately with no refund.

□ Cancellation Policy for the Summer Trainings in Europe: Early Bird registration: No refunds.
Before 12 weeks prior to start date: full refund minus $1000 deposit.
12 weeks or less from start date: No refunds. For students on payment plans - there are no cancellations and no refunds allowed. The full payment is due, whether you come or not.
Shamanic Journeys: NO REFUNDS.

□ I understand that all FEEs cover tuition, food, lodging and the LVL II workbook. Not included is transportation to and from the center. Level I Test is optional and extra. Finding your fundamental note with Fabien is also optional and extra.

  transfer, check or cash. If there is a bank charge for the wire transfer, I will pay this as well.

• ____I will pay a $1000 non refundable deposit due by February 15, 2016.
• ____I will pay the balance of $3500 in full by March 15, 2016.

____I understand that reservations are not firm until full payment is received in full and all RSVP Contracts as well as the Tama-Do Code of Conduct are filled out and received by the Academy, no later than March 15, 2016.

ADDITIONAL FEES
